STEP 1: Select Your Payment Amount
Review your plan level below and enter the exact total into the payment portal on the right:
🛡️ Individual Annual Plan: $379 / year
Covers 2 hygiene cleanings, exams, X-rays, emergency visit + 15% off all general care & aligners.
👨👩👧 Additional Adult / Family Member: $299 / year
Spouse, partner, or adult dependent.
🧒 Child Annual Plan (Ages 12 & Under): $225 / year
Includes cleanings, exams, X-rays, fluoride treatments, and preventative care.
🩺 Periodontal Maintenance Plan: $579 / year
Designed for gum health support—includes 4 periodontal maintenance cleanings, exams, X-rays, emergency visit + 15% off additional care.
STEP 2: Enter Portal Payment Details
In the payment window on the right:
Statement ID: Leave this blank (optional field).
Patient Details: Enter the primary plan member’s Last Name and Date of Birth.
Cardholder Details: Enter your Zip Code, Phone Number, and the exact Payment Amount for your plan selection.
Complete Payment: Click the blue Make Payment button and enter your card info on the final confirmation screen.
STEP 3: What Happens Next?
📞 We’ll Call You to Finalize: Once your payment is received, a member of our Blue Wolf Dental team will call the phone number provided to welcome you, confirm covered family members, and schedule your first preventive care visits!
